You Can Help Improve Lives!

Please give patients the dignity of remaining in familiar surroundings by contributing to the Bridgeport Hospital Home Visit Program.

For over eleven years, hundreds of families and patients have been helped with a variety of personalized health care essentials, ranging from routine primary care services to critical care assessments at home. Our Board Certified geriatricians and Palliative Care physicians are assisted by Case Managers who ensure that all pharmacy and other needed support is available and delivered in a timely and efficient manner, assuring continued quality care and reducing stress on the family.

Unfortunately, this compassionate and vital service can only continue with philanthropic support. Although a portion of the costs to provide this service are reimbursable by Medicare and some supplemental coverage, today’s reimbursement levels have dropped and approximately 30% of costs are not covered.
